Medium-chain triglycerides are generally considered safe with a low risk of toxicity. Common side effects include gastrointestinal symptoms, such as diarrhea, abdominal pain, and flatulence. Gastrointestinal symptoms may worsen with increasing doses (diarrhea: ۲۰+ grams; abdominal cramping: ۵۰+ grams). Caution is warranted with supplementation during lactation because there is little safety data, although harm seems unlikely. Research on the safety of supplementation during pregnancy is lacking, and avoidance may be warranted because of the potential risks associated with elevated ketone levels during pregnancy. There are no established drug interactions with medium-chain triglycerides.

Medium-chain triglycerides (MCTs) are generally considered to be safe and well tolerated. Gastrointestinal complaints, diarrhea in particular, are the most commonly reported adverse effects, and diarrhea seems to worsen as doses exceed ۲۰ grams. In animal studies, MCTs have shown a very low risk of toxicity related to reproduction, organ health, immune function, and cancer.

Gastrointestinal symptoms	Route: Oral Dose: Symptoms may worsen with increasing doses (diarrhea: ۲۰+ grams; abdominal cramping: ۵۰+ grams). Notes: Diarrhea (mild to severe), flatulence, abdominal pain/cramping, bloating, belching, nausea, vomiting. Symptoms may be improved by taking MCT with food or by dividing the dose throughout the day, although the benefits of the latter have not been directly observed in studies.
Increased triglycerides	Route: Oral Notes: Small increases (approximately ۰.۱۴ mmol/L or ۱۲.۴ mg/dL). Likely not clinically relevant outside of dyslipidemia.

Medium-chain triglycerides have a low risk of drug interactions. To date, no interactions have been established.	Agent Effect Level of Evidence Sodium-glucose co-transporter ۲ (SGLT۲) inhibitors Sodium-glucose co-transporter-۲ (SGL۲) inhibitors — such as canagliflozin, empagliflozin, or dapagliflozin — are medications used to lower blood sugar levels that can increase the risk of euglycemic diabetic ketoacidosis (a diabetes complication involving reduced body pH, high levels of ketones, and normal blood glucose). Medium-chain triglycerides can increase ketones and contribute to a state of ketosis, which theoretically might increase the risk of diabetic ketoacidosis when combined with SGL۲ inhibitors; however, this has never been reported and requires further study.
